JC Sports Development Ltd (JCSD) will collect and hold personal contact details including childs name, date of birth and gender. I understand that by ticking YES I consent to JCSD storing this information.

JCSD will collect and hold information regarding relevant medical conditions and/or disabilities and additional related information and any other relevant individual needs. I understand that by ticking YES I consent to JCSD storing this information.

This information will be stored by the Head Coach and used for the purpose of contacting you to confirm arrangements, to notify you about any changes, to tell you when it is time to renew membership or re-register for activities and to request and process payments. I understand that by ticking YES I consent to JCSD storing this information in order to contact me.

Information held by JCSD will be held until no longer required and will be deleted/destroyed without delay. (*Medical report forms will need to be kept for a minimum of 3 years). Data held by JCSD will not be held or used by any third party. I understand that by ticking YES I consent to JCSD storing this information.
